About DVA-Funded Community Nursing at Home

Who Can Access DVA Community Nursing?
White Card Holders
May be eligible for nursing services related to their accepted conditions. Our team can assist with confirming eligibility and available supports.
Gold Card Holders
Eligible to receive DVA Community Nursing services for clinically assessed care needs, subject to DVA guidelines.
How to Make a Referral
A referral from an authorised healthcare professional is required to access DVA Community Nursing services.

Complete and submit the official DVA Community Nursing Referral Form (D9389).
Healthcare Provider Letterhead
Authorised referrers, including General Practitioners (GPs) and other approved healthcare professionals, may submit a referral using their official letterhead.
